Pablo Lucio Vasquez, 38, was executed on Thursday for the murder of David Cardenas in April 1998.
He told police he was drunk and high when voices told him to kill the child during a party in the Texas border town of Donna.
The youngster was struck on the head with a pipe and his throat was slit.
The killer, who was 20 at the time, told investigating officers at the time that he heard voices telling him to drink the dead boy's blood.
"The blood was dripping and (I) got it all over my face, so I don't know. I mean, something just told me drink," he said, according to police transcripts of a confession.
David's body was found missing limbs and nearly decapitated in a vacant field five days after the murder.
Vasquez was found guilty of the killing after a trial.
Joseph Orendain, the lead trial prosecutor, described the case as "really horrendous", adding: "Did he drink (the blood)? I don't know."
Asked by a warden if he had a final statement before he was given the lethal dose of pentobarbital, Vasquez thanked relatives for being present, before addressing four members of his victim's family.
"I'm sorry to David's family," he said. "This is the only way that I can be forgiven. You got your justice right here."
Vasquez's execution is the 11th this year in the US and the sixth in Texas.
It took place four hours after the US Supreme Court rejected an appeal to block the execution.
His cousin Andres Rafael Chapa, then 15, who had used a shovel to help Vasquez, pleaded guilty to murder and is serving a 35-year jail term.
